The Camden Integrated Primary Care Falls Service provides two levels of falls prevention exercise groups: the Staying Steady exercise programme and the clinical referral scheme for physical activity. Patients will be directed to the group that is most appropriate for their ability.
This eight week exercise programme is for patients who may have poor mobility, muscle strength or balance, and are at risk of falling.
It comprises of group exercise and education designed to improve balance, strength, confidence and independence. It also includes discussion groups led by health professionals such as physiotherapists, occupational therapists, pharmacists, nurses, dieticians and psychologists.
The exercise group is for Camden residents who:
- Are unsteady, have fallen, have a fear of falling, or are at risk of falling
- Can independently transfer and mobilise a short distance with or without an aid
- Are able to follow instructions (cognition, vision, hearing)
- Are physically able to participate in an hour-long exercise session
- Are able to carry over learning from sessions to everyday activities
- Are medically stable
The service accepts referrals from GPs and internal referrals for patients who have completed the Staying Steady exercise programme class and are able to progress to more advanced exercises.
The service accepts referrals from the Integrated Primary Care Service in Camden, GPs and other health and social care professionals can refer to the Integrated Primary Care Service for a comprehensive falls assessment. Following this assessment, the therapist treating you (physiotherapist or occupational therapist) will refer you to a falls prevention exercise group, if appropriate.
