Great Chapel Street Medical Centre is to relocate to the nearby Soho Centre for Health and Social Care on the 18 February 2022 and open for patient care on 20 February 2022.

Great Chapel Street Medical Centre is a specialist GP practice that has been commissioned by NHS North West London Clinical Commissioning Group to provide care to the homeless population of Westminster. The current patient list size is approximately 300.

The decision to move to Soho Centre for Health and Social Care was taken as the landlord of the current premises had served notice on the practice to allow for redevelopment of the site. Patients will continue to see the same GPs, and health professionals as they do now.

The practice will be moving to a much improved accommodation that will support the specialist nature of the service and ensure long term sustainability. These new improved facilities will enable the practice to continue to provide excellent care in a fit for purpose comfortable environment for both patients and practice staff alike.

As the practice population is homeless, we will not be writing to patients in the normal way. Instead, we have agreed with the practice that they will engage with their patient population and supportive material has been developed to assist in this. See the supportive material below:
