Hillingdon Community Mental Health Hub, bring together a range of health care professionals who have specialist skills in working with people who have mental health needs. The service aims to deliver excellent, integrated, recovery focused and evidenced based mental health care to the residents of Hillingdon.

The role of the team includes:

  • Assessment, diagnosis and treatment of your mental health difficulties
  • Recovery-focused intervention
  • A wide range of psycho-social support
  • Support for families and carers
  • We work in partnership with services in the London Borough of Hillingdon, e.g. MIND, COVES, H4ALL and Adult Social Care
  • Work closely with local GP’s to provide secondary mental and physical healthcare
  • Provide a Mental Health Practitioner within every GP Practice.

This service is for adults (18+) who are resdients or registered with a GP in the London Borough of Hillingdon.

Routine referrals are through General Practitioners (GPs) with an expected time frame of 7 days – 28 days. At the moment this service does not accept patient self-referrals, patients are asked to speak to their GP to be referred.
