Camden Community Heart Failure Service helps patients manage their condition, encourage early recognition of their symptoms and reduce preventable hospital admissions by providing them with a comprehensive assessment and individualised care plan.
The service is for people who are over 18, registered with a Camden general practitioner (GP) and who have a diagnosis of heart failure from a secondary care setting and evidenced by echocardiography.
The service is not suitable for people who require diagnosis or complex interventions that are normally only available from secondary care.
The service accepts referrals from secondary care, GPs and other health and social care professionals.
Referrals are triaged by heart failure nurses and the Camden heart failure consultant. Discharged patients can also be referred back to the service.

The service is available from 9am to 5pm, Monday to Friday, and is provided at a number of community clinics across the London Borough of Camden. The service will also make home (domiciliary) visits if appropriate.
