The Camden, Islington ELiPSe and UCLH & HCA Palliative Care Service provide specialist care in community and hospital settings for patients with life-limiting illness.
Our palliative care specialists include nurses, doctors, occupational therapists, physiotherapists, social workers, clinical psychologists and assistant practitioners. Watch an interview with some of the team to find out more about the role of palliative care:
The team provides palliative care and advice to adults at home in the boroughs of South Camden, North East Westminster and Islington, and to teenagers and adults in University College London Hospital (UCLH) and HCA at UCLH. We also provide palliative care support for teenagers in UCLH.
The service is not suitable for children being cared for at home, as this service is delivered by the Lifeforce Team you can call them on 020 3316 1950.
The service accepts referrals from:
- General practitioners (GPs) and other health and social care professionals
- Acute services
- Community services
- Patients and families (but we will always check with the GP or hospital consultant to check that our input is needed before accepting the referral).
For community referrals, a North Central London Palliative Care referral form must be completed. GPs are welcome to include a brief EMIS summary.
The service is open Monday to Sunday 9am to 5pm (reduced service on weekends and bank holidays), with an on call service 5pm to 8pm.
Monday to Friday 9-5pm, any calls to the office will be picked up by our administrative team in the first instance, with clinical support available as required.
Weekends and bank holidays 9-5pm, please call the office and leave a message, which will be picked up periodically through the day.
Monday to Sunday 5pm-8pm, please call UCLH Switchboard on 020 3456 7890, ask for switchboard then ask the operator to contact the palliative care team, leaving a message for a callback
There is a separate telephone advice service available 8pm to 8am Monday to Sunday (provided by the North Central London Palliative Advice Team), which can be accessed on 020 8152 3252.
