CNWL  has a multi-agency pathway, called the Young Adult Mental Health and Wellbeing Partnership (the YAP).

The YAP aims to support professionals working with young adults presenting with a complex range of needs and/or where staff are unsure of the appropriate service to approach.

There is a Young Adult Lead in each borough who can provide support to professionals working with young adults aged 16-25 in getting the right care for them.

The YAP is available in the following CNWL boroughs:

  • Kensington & Chelsea
  • Westminster
  • Brent
  • Harrow
  • Hillingdon

We will facilitate mental health and wellbeing support to young adults (16-25) presenting with a complex range of needs and/or where staff are unsure of the appropriate service to approach. We will also review open cases in different services where there is concern about disengagement.

We provide consultation by email, over the phone or within a multi-agency YAP forum. 

At the YAP forum, you will give some background to the young adult and then the Young Adult Pathway Leads will help you develop a management plan. The meetings are open to any professionals working with young adults. 

YAP forum are held in all the boroughs on a fortnightly basis and are led by the Young Adult Pathway Leads in conjunction with the Young Adult Consultant Psychiatrists.
