The Brent Older Adult (OA) Home Treatment Team (HTT) provides intensive home treatment for older adults at home and aims to prevent unnecessary hospital admission as well as help facilitate early discharge from hospital through prompt and expert assessment, treatment and care delivered by a multi-disciplinary team.
- This service is for people living in the London Borough of Brent.
- People under the care of Brent OA Mental Health Services.
- People with mental disorders and significant physical illness, where those physical illnesses are related to the ageing process and/or impact the management of their mental disorder. Only exceptionally this may include people under 60
- People with psychological or social difficulties related to the ageing process, or end of life issues, or who feel their needs may be best met by an older adults and healthy ageing service. This would normally include people over the age of 70
- People with confirmed diagnosis of dementia (of any age). Please note we do not automatically accept with Alcohol Related Brain Injury but will discuss on a case to case basis.
Please note when making referrals to OA-HTT out-of- hours, the above referral criteria are still valid. Being over 65 years of age does not automatically mean that individuals will be taken on by the Older Adult OA-HTT. In exceptional circumstances, transfer between HTT can happen via a medical lead conversation and on a case-by-case basis.

Please note the CMHT does not ordinarily use CNWL’s Single Point of Access (SPA). Patients may still access the urgent advice line like all other CNWL service users, but referrals are not made through SPA. All referrals (including OA HTT) should be sent to firstname.lastname@example.org during normal working hours (Monday to Friday, 9am to 5pm).
Between 5pm and 7pm on weekdays, 9am to 5pm every other weekend and bank holidays. Urgent referrals can be made to OA-HTT directly via phone 0208 206 3920 and email email@example.com.
Any urgent referrals received after 6:30pm, and in the event of crisis the team will contact the Referrers to discuss the case. In the event the crisis cannot wait for intervention until the next working day, the referrer will contact SPA or the Central Flow Hub if admission to hospital is required and the HTT team will be informed of the admission as soon as possible or the next working day.
Who can Refer?
We accept referrals from GPs, Psych liaison, inpatient services and other healthcare professional. If referrals are not received directly from a GP practice we will inform them of the referral and request updated information if required.
Important Information to include on Referrals
- Detailed reason for referral to mental health services and whether the person is aware of the referral.
- Any associated risks e.g. self-harming behaviour, aggression etc.
- Full contact details of next of kin or significant relatives or carers.
- Relevant brief summary of past psychiatric and medical history (including current medication).
- Recent delirium screen and relevant investigations, e.g. dementia screen, scan results etc.
Monday to Friday: 08:00-20:00 (if the HTT is not able to respond to the crisis required within the remaining working hours the referral may be passed to the out of hours service and then assessed with the HTT)
Every other Saturday to Sunday: 9:00-17:00
The team are based at Fairfields House in Kingsbury, Brent. This service visits patients at their homes.
