The Milton Keynes Home 1st Service is a jointly delivered service between CNWL and Milton Keynes Council that provides fast access to nursing, therapy and reablement to help you to remain safe and supported in your own home or usual place of residence.
If you’ve been referred to us you may have been unwell recently or have been in hospital. You may need support to help you to stay at home or come home from hospital.
We are a 24-hour service providing you with a full assessment by a member of the team at your home to help you effectively manage your health and social care needs. This may include nursing or therapy support or personal care for a short period of time at home.
Your GP will be responsible for your ongoing medical care.
The team includes nurses; therapists; social workers, reablement assistants; health care assistants; speech and language therapists, psychology services and a Senior Medicines Optimisation Technician.
The service is for people who have been referred to us and who are registered with a Milton Keynes general practitioner (GP).
The service accepts referrals from secondary care, GPs and other health and social care professionals.
The service is open Monday to Friday from 9am to 5pm (closed on public holidays)

- Contact the CNWL service looking after you during their office hours
- If the crisis is during when your service is closed, there are alternative options provided by the NHS found on this page.
- If you are not already being looked after by CNWL or elsewhere within the NHS, see your GP during normal working hours as they can then refer you to a local service.
