Completing the Electronic Consent Form for Your Child's School-Aged Immunizations
Vaccinations are a very important way to protect your child and the wider community against some serious and potentially fatal diseases. In order for these vaccinations to be delivered, permission is required from the child's parent or legal guardian. The School Immunisation Team now use online consent forms. This is a quick and simple process.
To begin, please locate your child's school by typing the name of the school in the search bar above the list of schools. Click on the corresponding link to the right, which will redirect you to the online ParentPortal. There, you can easily indicate your consent for the vaccination.

Please note: If accessing via laptop or desktop, please don’t use Internet Explorer, instead use other internet browser such as Chrome.
How to search for your child's school
For best results, please type in the name of your child’s school in the ‘School name’ section.
If you need to narrow your search, you can select the borough, school year and campaign. However, please note if a particular campaign isn’t showing for your school year, please try selecting ‘reception to year 11’.
Hints and tips for your search
If you're struggling to find a school in the search, please try using key words instead of typing the name of the full school. For instance, instead of 'St Paul's', you might want to typing in 'paul'.
